your safety and will be kept completely confidential. If you do not want
to answer any question(s) on this form you are free to withdraw from
this scanning session without any prejudice."

Please read this text to potential participants before you ask the questions on the form, some of which can be personal in nature and which some individuals may not wish to answer.
