Participant Registration and Consent Forms

Before February 2014 (current)

• YNiC Participant Registration Form

  • The YNiC Participant Registration Form must be filled in by anyone who wants to either participate in a neuroimaging study at YNiC, or join the YNiC Volunteer Pool.

• Full Consent Forms

  • The full YNiC consent forms. This contains the General and Scan consent forms along with the safety questionnaire.

From February 2014 (future)

• YNiC General Consent Form (version 1)

  • YNiC Registration and General Consent form. Every participant must have completed one of these.

• Session consent forms

  • Note: It should not generally not be necessary to download these forms. If participant details are given to YNiC in advance (via the booking system), personalised forms will be prepared in advance of the scan.
