Participant Registration and Consent Forms

Before February 2014 (current)

• YNiC Participant Registration Form

  • The YNiC Participant Registration Form must be filled in by anyone who wants to either participate in a neuroimaging study at YNiC, or join the YNiC Volunteer Pool.

• Full Consent Forms

  • The full YNiC consent forms. This contains the General and Scan consent forms along with the safety questionnaire.

From February 2014 (future)

• YNiC General Consent Form (version 1)

  • YNiC Registration and General Consent form. Every participant must have completed one of these.
• Session consent forms.

  • Note: These should generally not be necessary. If participant details are given to YNiC in advance (via the booking system),

    • personalised forms will be prepared in advance of the scan.
